GDPR advice for EDI surveys: Your data, our responsibility

This survey will help us to understand how equal, diverse and inclusive we are being as an organisation from lots of different perspectives and experiences.

Following the EDI survey will be some diversity demographics questions where we will ask you to tell us about your identity, including information about your:
– ethnicity
– religion or belief
– physical and mental health
– sexual orientation

For each of these questions, you can choose one of the options provided, enter your own option, or indicate that you  would “prefer not to say”. You can also choose not to answer these questions.

We will not ask for your name, date of birth, address or contact details in this survey.

Your answers will only be viewed all together by Oxfordshire Mind’s Data and Quality Service Manager and EDI Lead, unless there is a legal obligation to share further.

Data that has the potential to identify individuals will not be shared in the reported findings featured in Oxfordshire Mind’s EDI strategy.

To find out more about GDPR and Oxfordshire Mind, please contact our expert, Debbie:

You are completing this survey via a platform called SmartSurvey. Any information you share here will be stored on SmartSurvey’s cloud, which is fully GDPR compliant with data stored only in the UK and Ireland.

Your responses will be retained for 12 weeks after the survey closes and they will then be permanently deleted.
