Community Connectors

Berkshire West – Wokingham, Reading and West Berkshire

Community Connectors work within the NHS MHICS Teams (Mental Health Integrated Community
Service), multi-disciplinary teams which work locally to support clients to
stay healthy in their communities.

Community Connectors support clients to access a wide range of community services and resources including social care, housing, family, debt and employment counselling that support maintaining good health and wellbeing. Community Connectors don’t provide clinical intervention but a holistic approach.

It’s important to highlight that this service is not an open referral team and referrals come through the MHICS Team directly. There are three MHICS Teams working across the Berkshire West:

Reading MHICS Team

Wokingham MHICS Team

West Berkshire MHICS Team
